LYMAN, SC (01/23/2017) – A woman was killed and two others are in the hospital following a crash on Hampton Road near Lyman Friday evening, according to South Carolina Highway Patrol and the Spartanburg County Coroner’s Office.

SC Highway Patrol reports that the collision happened around 7:08 p.m. near Skyway Ct. after an SUV headed south crashed into at least one vehicle headed north, involving a third vehicle.

The driver of the SUV headed south has been identified as Lauren Smith, 31, and she was charged with one count of felony DUI resulting in death and one count of felony DUI resulting in great bodily injury, says Highway Patrol. Smith was taken from the scene by EMS for treatment.  MORE

Reports say the driver of the third vehicle, the Honda, was not wearing a seat belt and was entrapped. She died on the scene, troopers say. A male passenger in the vehicle was also not wearing a seatbelt and was taken to Greenville Memorial Hospital for injuries, according to troopers. No word on his condition.

The coroner identified the deceased victim as 53-year-old Viola Lynn Jackson of Hammett Store Road in Lyman.

01/24/2017 – A Carbondale man who drunkenly drove the wrong way for more than four miles on the interstate before crashing and killing another man pleaded guilty to three charges Monday.

Adam David Kovaleski, 32, appeared before Judge Margaret Bisignani Moyle clean-shaven and wearing a shirt and tie. Prosecutors had charged him with a long list of crimes, but agreed to drop several in exchange for his guilty plea to felony vehicular homicide while DUI, misdemeanor DUI and driving the wrong way, a summary offense.

Per state law, the vehicular homicide while DUI conviction comes with a mandatory minimum of three years in prison, Judge Moyle told him.

MILFORD, Pa. – Timothy J. Kunis, 55 of Milford, Pa., went to heaven to be with his mom, dad, sister and pets on Oct. 15, 2015, in Lackawanna County, Pa. He was a member of Holy Trinity Lutheran Church and was a pressman for the Scranton Times.
He had a sarcastic, yet a kind sense of humor. As the antagonist of the family, his antics were only outweighed by his warm-hearted intentions. He loved cigars, football, especially the Kansas City Chiefs, rock ‘n’ roll music, horror movies and his man cave.
The son of the late John and Margaret (Hoey) Kunis, he was born in Prospect Park, on June 26, 1960. Timothy was married to Elizabeth Scholtes. Survivors include his wife; five children, Adam, Michael, Elisa, Anastasia and Michele, and their spouses; four grandchildren, Jordan, Shane, Carter, and Maci; four siblings, John, Fred, Jane and Amy. He was predeceased by his sister, Carol.
Friends may call at Holy Trinity Lutheran Church, 19 Delaware Crest, Dingmans Ferry, Pa., on Monday, Oct. 19, from 3-8 p.m. A service will be held at Holy Trinity on Oct. 20, 10:15 a.m., with the Rev. Niels Nielsen officiating.

The Sarasota County Sheriff’s Office arrested a Manatee County couple after first responders found them passed out in a car with two infants in the backseat.

Deputies were called to the Texaco gas station located at 19 East Road, Sarasota, just after 8:00 a.m. on Thursday, for the report of two possible overdoses. When deputies arrived they located driver William Ballard, DOB 02/02/80, and passenger Delaney Crissinger, DOB 04/28/84, passed out in the front seat of a black SUV with the engine running.

Crissinger was holding a clear baggy containing .61 grams of methamphetamine and after a search of the vehicle, deputies recovered another nearly 2 grams of the drug, .38 grams of fentanyl and heroin, baggies, a scale, and spoon. A loaded syringe was also found on the floorboard next to a container of baby formula.

Two infants, ages 5 and 18 months, were secured in car seats in the backseat. Crissinger was holding a clear baggy containing .61 grams of methamphetamine and after a search of the vehicle, deputies recovered another nearly 2 grams of the drug, .38 grams of fentanyl and heroin, baggies, a scale, and spoon. A loaded syringe was also found on the floorboard next to a container of baby formula. The couple was arrested and taken to the Sarasota County jail.

Ballard is charged with Child Neglect, Possession of Heroin with Intent to Sell, Possession of Methamphetamine with Intent to Sell, and Driving with a Suspended License. He is currently in custody on $30,120 bond. Crissinger also faces charges of Child Neglect, Possession of Methamphetamine and Possession of Narcotics Equipment. During a first appearance hearing, the judge set her bond at $17,000.

NEW YORK — Officers with U.S. Immigration and Customs Enforcement’s (ICE) Enforcement and Removal Operations (ERO) report that agents arrested a Mexican national Wednesday, Jan. 26, 2017, after he was released from local custody when a detainer lodged with New York authorities was not honored. The man, who had previously been removed, has a prior conviction for forcible theft armed with a deadly weapon.

Luis Alejandro Villegas, 31, was released from local custody Dec. 31, 2016, after being detained for driving while intoxicated. Villegas has a previous criminal conviction for robbery in the first degree, forcible theft with a deadly weapon for which he served five years’ imprisonment.  Following his prison term, Villegas was removed from the United States in 2007 pursuant to a final order of removal.

New York City is one of many sanctuary cities where local officials refuse to cooperate with legal detainers filed by ICE. The city faces a possible cut-off of Federal funds as orders from the new administration of President Donald Trump take effect.

ICE filed the detainer Dec. 31, 2016, with the New York Police Department in Manhattan, where he was detained while the DWI charge was pending.

ICE officers arrested Villegas Jan. 25 at the intersection of 111th Street and Park Avenue in New York.  He was processed and later transported to federal court for an appearance before a judge.  He is now in custody of the U.S. Marshals Service with charges pending.

“Villegas is a criminal alien who was released back into our New York communities, posing an increased and unnecessary risk to those who live in this great city,” said Thomas R. Decker, field office director for ERO New York.

On Thursday, January 19, 2017, at approximately 10:00 a.m., Nevada Highway Patrol Dispatch received a call from a witness about a possible rolling domestic violence call inside a vehicle in the area of Interstate 580 and Moana. The vehicle was described as a red Hyundai sedan. A Nevada Highway Patrol Sergeant located the vehicle going southbound on IR580/Mt Rose Highway. The Sergeant initiated a traffic stop using his emergency lights and sirens. The red Hyundai sedan failed to yield and stop for the emergency vehicle and continued south. The pursuit continued southbound on US395Alt through Washoe Valley. The vehicle turned onto eastbound Eastlake and got back onto the northbound IR580 on ramp.

A Nevada Highway Patrol Sergeant located the vehicle going southbound on IR580/Mt Rose Highway. The Sergeant initiated a traffic stop using his emergency lights and sirens. The red Hyundai sedan failed to yield and stop for the emergency vehicle and continued south. The pursuit continued southbound on US395Alt through Washoe Valley. The vehicle turned onto eastbound Eastlake and got back onto the northbound IR580 on ramp.
Police reported that due to the danger the driver of the Hyundai posed to the public, the Nevada Highway Patrol Sergeant performed a PIT maneuver causing the red Hyundai to collide with the concrete barrier and come to a stop on the left side of the road.

Police say the driver of the Hyundai exited the vehicle and fled on foot across both directions of IR580. Nevada Highway Patrol Troopers chased the man on foot and were able to take him into custody. Two Troopers sustained minor cuts from jumping over the barbwire fence. They were treated on scene and released. The driver was identified as Tyrone Thomas Preciado Sr. of California. He was taken into custody and transported to the Washoe County jail and booked for Reckless driving, DUI Drugs, Eluding Law Enforcement (Felony), and Domestic Battery. Both the driver and the female passenger were unharmed in the crash. The female passenger was not arrested and was released.

LAS VEGAS, NEV.  –  The Nevada Highway Patrol has charged

The Clark County Coroner’s Office has identified Jaelan Jonson Fajardo as the teenage boy killed by the four-time DUI driver at the intersection of Durango Drive and Farm Road.

Police report that the crash took place on Feb. 9, 2017, around 6:15 a.m.  Nevada Highway Patrol investigators say David Fensch was driving a Chevy Trailblazer and rear-ended the teen’s Honda, which was stopped at a red light, at a high rate of speed.

Both drivers were transported to University Medical Center. The teen died at the hospital, investigators said.

Fensch, who police have charged with impairment, received only minor injuries, according to the Nevada Highway Patrol.

Charges against Fensch include vehicular homicide, failure to reduce speed to avoid a collision and driving without a license.

Fensch was treated and released from a hospital after suffering only minor injuries and was then incarcerated at the Clark County Detention Center.

News 3 is learning new details into a deadly crash that took the life of a 16-year-old Shadow Ridge High School student on his way to school.

Law enforcement sources say right before the crash that took the life of Jaelan Fajardo, 47-year-old David Fensch was drinking at Bally’s Casino.

Fensch admits to police he drank four glasses of rum and also was on Xanax. It’s a drug used to treat anxiety. After the crash, sources say Fensch smelled of alcohol and slurred his speech.

Sandy Heverly with STOP DUI is upset over the deadly crash, “We are curious to know why he ever had access to that vehicle. How did that come about? He shouldn’t have been near one much less behind the wheel.”

Heverly is well aware of the previous DUI’s Fensch was charged with.

Court records show Fensch was charged two times with DUI in Michigan and two times here in Clark County.

District Attorney Steve Wolfson was also upset over the crash. Wolfson said, “His conduct is unacceptable, but I blame in part this community. Whatever we are doing isn’t working to get the message out that it is unacceptable behavior driving under the influence.”

District Attorney Keith Stutes announces the conviction of Bryce M. Chapman of Vehicular Homicide and three counts of Vehicular Injury.

On Mardi Gras Day, February 17, 2015, the defendant was operating a motor vehicle while under the influence of alcohol while traveling in excess of 100 miles per hour when he struck Carol Richard’s vehicle causing a fire and explosion killing Mrs. Richard. The vehicles then collided with a vehicle occupied by Karen, Christina and Cynthia Escavage causing injuries. Chapman was sentenced on November 3, 2016, by Judge David Smith. The defendant faced up to 30 years hard labor but got only 5 years in prison. The case was prosecuted by First Assistant District Attorney Daniel M. Landry III and was investigated by the Lafayette Police Department.

A Lafayette man convicted of driving drunk and killing a woman in a 2015 crash was sentenced Thursday to serve five years hard labor.

Bryce Chapman, 25, was handed a 12-year sentence, with all but five of those years suspended, on a count of vehicular homicide in the death of 66-year-old Carol Ann Richard. He was also sentenced to serve six months each on three counts of vehicular negligent injuring — to be served concurrent and concurrent with his vehicular homicide sentence — as the crash involved a third car with three people inside.

Chapman pleaded guilty to the crimes in July, and he’s been held in the Lafayette Parish Correctional Center since.

During an emotional court hearing, 15th Judicial District Judge David Smith heard testimony from seven of Richard’s close friends and family members who, through tears, described the late, longtime Lafayette Parish School System employee as a beloved woman, artist and giving soul. The woman who drove the third car in the accident also testified, and she’s requesting restitution be paid to cover medical bills for her and her two passengers.

Chapman’s parents also took the stand. They testified that their son had maintained sobriety since the crash and described him as a caring brother to his special needs sister. Chapman also took the stand and apologized to those affected by his crimes.   MORE

The driver of the Camaro, 23-year-old Bryce Chapman of Lafayette, was arrested and booked into the Lafayette Parish Jail on charges of OWI second offense, first-degree negligent injury, two counts of vehicular negligent injury and vehicular homicide.

Chapman’s first OWI arrest was about five months ago in September 2014, when he was accused of backing his Camaro into a vehicle in the Cowboys parking lot and leaving the scene.

Scott Police pulled Chapman over at the corner of Ambassador Caffery and Renaud Drive. Chapman reportedly agreed to a field sobriety test that registered a .18 blood alcohol concentration.  MORE

n February 6, 2017, at approximately 7:00 am, Lafayette Police responded to the Fatima Church at 2319 Johnston Street in reference to a vehicle that had struck the church building. The driver of the vehicle, Christopher Guillot (11/4/69), was arrested for O.W.I and Aggravated Criminal Damage to Property after he drove his vehicle into the front doors of the Fatima church building. Narcotics were a factor in the crash. No one was injured in the crash.

The incident happened just a few minutes before students and parents began arriving at the adjacent Our Lady of Fatima Catholic School.

Principal Herb Boasso said he already was on campus at the time.

“I was checking to make sure it was isolated and not something we needed to worry about for the school,” Boasso said. “We were able to get a message to the parents to let them know that we would operate as normal because they were already calling and were worried about it. Zoller (A) 28 YOM, Flanders, NJ On 2-4-2017 at 3:17 am, Officer Michael Sciscione was on patrol on Route 46 when he observed a 2008 blue Subaru with a broken headlight travelling at a high rate of speed. Officer Sciscione began to follow the vehicle when he observed it failing to maintain the lane. He stopped the vehicle on Route 46 and Anderson Place. Upon speaking to the driver, identified as Maxwell Zoller, he noticed his hands to be fumbling. He asked Zoller to exit the vehicle and upon speaking to him, Officer Sciscione noticed an odor of alcoholic beverage emanating from him. Zoller was asked to do standardized field sobriety tests. Upon completion of the tests, Zoller was placed under arrest for driving while under the influence of alcohol. Zoller was transported to headquarters for processing where he submitted to chemical breath testing.  Zoller was charged with DWI, reckless driving, failure to exhibit documents, speeding, failure to maintain lamps and failure to maintain a lane. Zoller was released to a sober driver pending court.

On 2/6/2017 at 1:11 am, Officer Lonnie Elbaum was on patrol on Route 46 when he observed a vehicle traveling Route 46 West with the high beams on, failing to dim the lights with oncoming traffic.    Officer Elbaum followed the vehicle onto Old Wolfe Road and conducted a motor vehicle stop. On February 3, 2017, at 12:20 am, Officer Mullooly was on routine patrol when he observed a 2011 green GMC traveling west on Rt. 46 fail to maintain its lane of travel on multiple occasions, striking the curb in the area of Woodsedge Avenue. Officer Mullooly conducted a motor vehicle stop of the GMC in the area of the Budd Lake beach. Officer Mullooly approached the driver, who was identified as Moorehead. As Officer Mullooly spoke with Moorehead he detected the odor of an alcoholic beverage and noted that Moorehead appeared to be impaired. Moorehead was asked to step out of the vehicle to perform field sobriety tests. Moorehead was subsequently placed under arrest and transported to police headquarters, where he submitted to chemical breath testing. Moorehead was charged with DWI, Reckless Driving, and Failure to Maintain Lane. Moorehead was released to a sober driver pending a court hearing.

On January 23, 2017, at 11:25 pm, Officer Gardner responded to Sand Shore Road for the report of a vehicle off of the roadway. When Officer Gardner arrived on scene he observed a 2013 red Volkswagen off of the roadway, with the front end pinned against a tree. The driver of the Volkswagen was no longer at the scene. Through the course of the investigation, the driver of the
vehicle, identified as Burghardt, was located at a residence on Winding Hill Drive in Mt. Olive Twp. Officer Gardner made contact with Burghardt, and determined that he was uninjured in the crash, but appeared to be impaired. As Officer Gardner spoke with Burghardt he detected the odor of an alcoholic beverage, and Burghardt was unsteady on his feet. Burghardt was subsequently placed under arrest, and transported to police headquarters, where he submitted to chemical breath testing. Burghardt was charged with DWI, Reckless Driving, Leaving the Scene of an Accident, and Failure to Maintain Lane. Burghardt was released to a sober driver pending a
court hearing.

Conrad Oliver Antonio Thorpe 23 of Waterwheel Square in Randallstown Md. DUI homicide

TOWSON, MD.  Baltimore County Police report that a DUI driver killed a boy on a bicycle. The crash happened just before 9 p.m. on Jan. 27, 2016, near Marriottsville Road. The preliminary investigation revealed that a 19-year-old woman and a 14-year-old boy were riding together on one bicycle west on Liberty Road. They were struck by a 2002 Nissan Ultima, also traveling west on Liberty Road.

The boy, who is identified as Jamal Quentin Anderson of the 3900 block of Red Deer Circle in Randallstown, was pronounced dead at the scene. The woman was transported to an area hospital with life-threatening injuries. She remains in stable condition at last report.

The driver of the Nissan, who did remain at the scene, is identified as Conrad Oliver Antonio Thorpe, 23, of 3640 Waterwheel Square in Randallstown, Md. He has been charged with Driving Under the Influence and is being held at the Baltimore County Detention Center without bail.

This case continues to be investigated by the Baltimore County Crash Investigation Team. Upon conclusion, the case will be forwarded to the State Attorney’s office for review regarding any additional charges.

ARRESTED: Conrad Oliver Antonio Thorpe, 23, of 3640 Waterwheel Square in Randallstown, Md.

Thorpe has been charged numerous times in Baltimore County with failing to provide a driver’s license on demand to an officer, failing to have tags or a permit with the charges all dropped by the Baltimore County States Attorney.

On May 1, 2014, Carroll County States Attorney Jerry Barnes gave a plea deal for DUI to Thorpe of a fine of $100 and no jail time in exchange for a guilty plea that resulted in Probation Before Judgement.

On Feb. 9, 2014, Thorpe was arrested for DUI by a Maryland State Trooper as Thorpe operated a 2011 Hyundai eastbound on Md. Rt. 26 west of Fallon Road in Carroll County, Md. On May 1, 2014, Carroll County States Attorney Jerry Barnes gave a plea deal for DUI to Thorpe of a fine of $100 and no jail time in exchange for a guilty plea that resulted in Probation Before Judgement.

Brian L. DeLeonardo is presently the State’s Attorney of Carroll County, serving since January 5, 2015. DeLeonardo was appointed Interim State’s Attorney, Dec. 1, 2014, to Jan. 5, 2015 to fill out the remaining month of Jerry Barnes’s term.

DeLeonardo became States Attorney after defeating the incumbent States Attorney Jerry Barnes, 66, in the GOP primary in 2014. Barnes then shot himself following the General Election in November of 2014, in which DeLeonardo was unopposed and was therefore elected to office.

Barnes had been States Attorney for twenty years after being elected in 1994 and worked as a prosecutor for 18 years prior to his election.

Man Arrested in Fatal Crash Investigation

North Las Vegas, Nevada – According to North Las Vegas police a Las Vegas man is dead following a crash that occurred on Feb. 12, 2017, in North Las Vegas.

On Sunday, February 12, 2017, at about 6:45 a.m., officers responded to the intersection of Centennial Parkway and Losee Road regarding a traffic collision involving two vehicles.  A 56-year-old man was transported to UMC Trauma where he later died. 

The preliminary investigation indicates a black Chrysler 300 was traveling westbound on Centennial Parkway when it failed to stop for a red light.  The Chrysler collided with a black Volkswagen Tiguan that was traveling northbound on Losee.  Investigators believe the Chrysler was traveling at a high rate of speed.  Impairment also appears to be a factor in this case. Two passengers from the Volkswagen were transported to UMC with non-life threatening injuries.  

The driver and sole occupant of the Chrysler, Alex Sadler was taken into police custody.   He was later booked at the Las Vegas City Jail on felony charges including DUI resulting in death and two counts of DUI with substantial bodily harm as well as other traffic related offenses.  

The decedent’s identity and cause and manner of death will be released by the Clark County Coroner’s office.

David Kriehn Killed in a Crash Caused by  Suspected Drunk Driver Elizabeth Vargas-Hernandez

Indianapolis, IN –02/14/2017 – Indiana State Police report that an early morning crash on I-465 on Feb. 13, 2017, has claimed the life of a Noblesville man. Just before 2:30 a.m. emergency crews were called to I-465 eastbound near Michigan Road for a serious crash. Upon the arrival of first responders, troopers found two inverted vehicles, one on the right shoulder of the interstate and one in the ditch.

Indiana Troopers report that 66-year old David Kriehn was the driver of the vehicle in the ditch, a red 2002 Honda. Kriehn was ejected from the vehicle as it rolled several times, he was rushed to St. Vincent Hospital where he later died from his injuries. The driver of the other vehicle, a 2009 Nissan Murano, was 35-year-old Elizabeth Vargas-Hernandez of Indianapolis, she was not injured. There were three passengers in the Nissan who were checked and released by medics at the scene.

Preliminary investigation and witness statements led police to believe Vargas-Hernandez was traveling at a high rate of speed in the right lane when she came upon the Honda. Police say that Vargas-Hernandez struck the rear of the Honda causing it to slide off the road and overturn several times in the ditch. The Nissan also overturned and came to upside down on the right shoulder.

Based on the investigation Vargas-Hernandez was arrested for operating a vehicle while intoxicated causing death. She was also cited for operating a vehicle without ever receiving a driver’s license. A blood draw was taken for blood alcohol content, those results are pending.

The charges listed in this release are merely accusations, the Marion County Prosecutor’s Office will make the final determination on actual criminal charges. All suspects are presumed innocent until and unless proven guilty in court.
